If TAFE QLD is producing media that people can be seen in, each person must fill out this form:Consent formone of these forms.

There are 2 versions of the form:

1. The general consent provides the right for the individual to withdraw their consent in the future by notice to us.
2. The Campaign Talent version excludes this as we cannot allow this where we have engaged someone for the purposes of a campaign. There may be other instances outside of campaigns where the “campaign” version would be more appropriate for use and this is absolutely fine.
